How to win back past patients without spamming them
Kept Team · Updated 2026-09-01.

Every practice holds months of unfinished business: consults that never booked, plans never finished, patients who drifted after one visit. That list cost thousands to build.
Winning them back isn't blasting it. It's cleaning it, checking consent, and sending short personal notes in small batches so every reply gets answered.
Clean before you send
Remove dead numbers, past complainers, and anyone who opted out. A smaller clean list always beats a big dirty one, on replies and on reputation.
Segment by recency and value: recent consults first, then unfinished plans, then patients who haven't visited in months. A different note for each.
Consent is the whole game
Every message carries opt out, every STOP is honored at once and suppressed. Carrier rules and the law agree on this, and so does basic decency.
Keep records of basis per list. If you can't show why someone was texted, don't text them.
Small batches, personal replies
Send to dozens, not thousands, so your team answers every reply the same day. A win-back note that goes unanswered is worse than silence.
Plan for single-digit revival rates: three percent of a thousand names is thirty visits from demand you already paid for.
Start with fifty names
Do not start with the whole patient list. Pull fifty recent consults that never booked, clean them, check consent, and send one short personal note. Count replies, bookings, and opt outs.
Fifty names teach you the message, the timing, and the measured revival rate for your list. Scale what worked to five hundred. Delete what didn't.
Questions.
How old can the list be?
Up to two years typically. Older than that, test a small sample first.
What should the message say?
Short, personal, one reason to return, one way to book. Then stop writing.
Can we text patients who never opted in?
No. Not for win-back, not once, not nicely worded. Collect consent at intake, on your site, and by email first. The list waits. The law does not.
What if our list has no consent records?
Then you have a collection job before a sending job. Start consent capture this week: intake forms, booking confirmations, website opt-in. Send nothing until the records exist.
The fix.
Patient Win-Back
Patients who never rebooked, welcomed back with consent.
